IMSI is basically a very advanced microscope with powers as much as x7200 times magnification. It is used to select the best sperm for ICSI procedure. Normally, in a conventional inverted microscope lens, when sperms are observed for ICSI procedure, the embryologist views them at only about 200 times magnification. There is a serious chance that the best sperm is left behind and a defective sperm is chosen to be injected inside the egg. Reason being, at 200-400 times magnification, one simply cannot see the fine structures that are visible at higher magnification. Some of the fine structures include vacuoles, abnormalities of the sperm head shape and size, etc defined according to the MSOME criteria. There is ample evidence that these subtle abnormalities in the sperm are associated with DNA defects and if these abnormal sperm are chosen to fertilise the egg, the resulting embryo would lead to sub-optimal or lower results. After an extensive analysis of the results, Dr. Dalal and Dr. Mishra found that comparing patients who availed IMSI versus those who didn’t, the patients who had sperms selected from IMSI technology fared better as far as pregnancy rates are concerned. Not only that, a very interesting phenomenon emerged from the study. On further sub-group analysis, we found that this IMSI procedure especially helps those who have had previous failures with IVF before. Not only it gave them a successful result, but also statistically had much lower miscarriage rate.
